Lilith’s backstory in the Diablo Universe
According to the game’s lore, Lilith is the daughter of one of the game’s three main evils: Mephisto. People who have played Diablo 2 or last year’s Diablo 2: Resurrected may be familiar with Mephisto, as he is the boss of the third act in that title. However, Lilith is the queen of the succubi and together with her father and her uncle, Diablo participates in the endless war with the high heavens.

And the two rallied like-minded followers to steal the Worldstone, creating Sanctuary, a world built between hell and heaven ruled by Inarius and Lilith, along with a council of demons and angels. This place acted as a respite for people who wanted a respite from the Eternal War.
The rise of the Nephilim and the purge
Eventually, Lilith will conceive children with Inarius, giving birth to Nephalem. These were the half-human, half-demon race that later became modern humanity. Eventually, other angels and demons also bred more Nephalems.
The first Nephalems, called Ancients, were far superior to angels and demons, impervious to ageing and disease. They began worshipping Lilith as a god and enacted altars around the Sanctuary. In the game, you may have come across these shrines in some cases. These were the ones.
